Payment & Insurance Information
 We will file your insurance as a courtesy to you. However it is your responsibility to know if we are on your insurance plan. Please refer to the telephone number on your card.
Co-payments are due at the time of service. There will be a $10 billing fee should we need to bill a co-payment. The parent who presents the child for treatment is responsible for the co-payment. We accept cash, check and credit/debit cards. (Master Card and Visa).
