Before Your First Appointment

We offer prenatal visits to help you prepare for your baby’s arrival, answer any questions, and introduce you to our practice. Our board-certified pediatricians, lactation support, same-day sick visits, and after-hours care ensure your little one receives the best care from day one.

When the happy day has arrived, you can tell the hospital staff that you have chosen Pediatric Associates of Mt Carmel as your pediatrician.  This will help the hospital forward your child's records to the right place. 

What to Bring
  • Any paperwork from the hospital, including hearing screen results and immunization records.
  • Any available insurance information (often only the parent’s insurance is available at the first visit).
  • Plenty of diapers and an extra set of clothes.
  • Completing the new patient forms help streamline your visit.
